When you schedule your first visit to our office we will need to know your previous provider as we will need to request any current xrays that they may have on file for you. This way, it will save you from having new xrays. In some instances, dental offices will require your written permission to release your xrays and may even charge a fee for duplication of them.
If you have a dental insurance plan we will require that you bring your insurance information with you to your dental appointment so that we can submit claims on your behalf.
If you have not been asked to fill out your new patient forms online, you will be asked to come approximately 15 minutes before your appointment time to fill them out.
